Do I need to contact my insurance company?

Yes.  First, to find out if pre-authorization, pre-certification or a referral form is required. 

Secondly, you should check to make sure your insurance company network lists Dr. Friedman and Charleston Orthopaedic Associates as being participating members with your network.  As networks frequently change and are purchased by other networks, our office may not have received information necessary to correctly advise you if we do or do not participate with your plan.  Some carriers have more than one plan type in their network; we may participate with a network, but the network may add or subtract plans with in their network with which we do not participate.

Lastly, to find out if you need a second opinion for surgery. If so, does the second opinion need to be by another surgeon? Failure to make this call may result in a reduction of benefits. 

What if I don't have insurance?

Please notify the Registration staff when you are called for pre-registration that you will need help in making payment arrangements. 

If I am a member of an HMO (Health Maintenance Organization), will my registration procedure be different?

It will basically be the same, however, you will need to call your Primary Care Physician (PCP) to arrange for the proper referral to be in place for your visit with Dr. Friedman.  This must be done prior to your appointment date and done by the PCP.  If you require surgery, our office will do what we can to obtain prior authorization for the procedure.  Certain plans require the PCP to obtain the prior authorization.  Your carrier may also need to speak with you before they authorize any services.

What information do I need to pre-register?

You'll need to have the following information ready:
  • Patient's full legal name and address
  • Home and daytime telephone numbers 
  • Date of Birth
  • Name of insurance holder, his or her home address, if different than the patient’s address, telephone number, and date of birth
  • Name of insurance company, mailing address, policy and group number
  • Patient's employer, address, phone number and occupation
  • Name, address and phone number of nearest relative or emergency contact person

We will need to know:

  • What type of problem you are having (what body part)?
  • Have you had any prior treatment by another physician?
    • If you have had prior treatment, you will need to contact that physician’s office and ask that your medical records be sent to Dr. Friedman for his review
  • Have you had any x-rays, MRI’s, or other tests taken of the injured body part?
    • If so, you MUST bring a copy of these films and test results with you to your appointment.  You may request copies of the films from the facility that took the films.  We can accept films that have been put on CD to be read off of a computer or actual x-ray/MRI films to be read on a view box.
  • Failure to bring the needed clinical information may result in a delay or rescheduling of your appointment.

What do I need to bring to my visit?

Please be sure to bring your medical insurance cards, picture ID, copies of any x-rays, MRIs, and other test results, any medical records from other physicians who have treated your problem, and a list of ALL medications and vitamins you are taking.

How much will my visit cost?

Each visit has different considerations that will effect how much you will be expected to pay at the time of service.  Each insurance plan has its own plan parameters.  You may have a co-pay, co-insurance, deductible, or combination of these.  You will be required to pay any applicable co-pays, co-insurance or deductibles at the time of service.  As each plan has
different benefit levels, any questions concerning how much your deductible, co-pay, or co-insurances are should be directed to your insurance carrier. 

Self pay patients are expected to pay at the time of service as well.  Please call our office prior to your appointment for an estimate of what your visit may cost.  The costs for our visits vary depending on the injury and any needed x-rays or procedures. 

For your convenience, we accept cash, personal checks, Master Card and Visa.
